As with any program there is always fine print, but the details are simple. We sought out the best way to reward our loyal patients who show up on schedule for their cleaning and periodic exams; whitening for life was the answer. After all most patients insurance covers exams, x-rays and necessary cleaning. As for patients without insurance we are constantly working on competitive fees, so that everyone has an opportunity to be a part of our dental patient family and our many programs. As promised we are happy to share each and every detail:
Dental Office Responsibilities:
For a one-time fee of only $99
1. Medical Arts Dental will make custom whitening bleaching trays for any of our current patients that are medically able to receive tooth whitening following a regular scheduled cleaning and exam.
2. We will take a custom impression for their teeth, and whitening trays will be ready for pick up within two weeks of the impression.
3. Our helpful staff will deliver custom made trays just for you, along with a supply of whitening gel for the initial whitening treatments. Patients will be given instruction on proper usage and give written instructions for teeth whitening.
4. Regardless of your cancelation history with us prior to March 01, 2009, we will allow all patients to participate in this great program, unless health conditions do not allow it. A clean slate is always nice for everyone.
5. As a reward for following the guidelines, Medical Arts Dental will give you a complimentery whitning touch up kit every six months for life!!!!!
Patient Guidelines and Responsibilities
1. You must become a patient of Medical Arts Dental and need to have a cleaning and exam prior to tray fitting appointment.
2. You must keep appointments for regular scheduled cleaning, exams, and necessary x-rays.
3. If a change comes up in your schedule, we require 24 hours notice so other patients can be served during your previously scheduled appointment. Also we do require that patients schedule within two weeks of previously scheduled appointment date.
4. Patient must agree that results vary between patients, and for some patients tray whitening may not be the treatment necessary for desired results, and that Medical Arts Dental is not liable financially or in any other way for your whitening experience. The patient must agree that we are not responsible for improper application or use. Patient will assume all financial responsibility for any expenses associated with results from improper use or application of whitening trays and gels.
5. If a patient were to have any negative health reaction to whitening gels, stop usage immediately, and contact your physician or dentist right away.
6. Failure to follow any of the above patient responsibilities may result in termination of the whitening for life program for that patient as we believe we have given every opportunity to properly participate and make necessary changes.
7. Medical Arts Dental ownership and management retain the right to review each patient’s program, and are able to stop program enrollment at anytime if deemed necessary. Also management agrees to take each case and look thoughtfully at any disputed details and holds final resolution authority.
8. If a patient is unable to remain in the whitening for life program, normal fees will apply for any future services associated with teeth whitening.
